About the Opportunity

The Nova Scotia Health Authority is seeking a skilled Care Coordinator to join our team. As a Care Coordinator, you will be responsible for assessing the needs of clients referred to Continuing Care and developing personalized care plans to support their health and well-being.

You will work closely with interprofessional teams to facilitate hospital discharge and ensure a smooth transition back to the community. Your expertise in community-based care management and ability to articulate and advocate for alternative solutions to address unmet needs will be invaluable in this role.

We are looking for a candidate with a strong background in nursing, social work, occupational therapy, physiotherapy, or nutritional studies, and a minimum of three years' experience in community-based care management. Your excellent assessment, interviewing, and negotiation skills, as well as your ability to work independently and co-operatively in a busy multidisciplinary situation, will be essential in this position.

About You
  • Bachelor's Degree in Nursing, Social Work, Occupational Therapy, Physiotherapy, or Nutritional Studies
  • Current and ongoing registration with the applicable Nova Scotia Professional Regulatory body
  • Minimum three years' experience in community-based care management
  • Demonstrated knowledge and/or experience in community-based care
  • Knowledge and/or experience in dedicated case management or discharge planning specific to caring for the unhoused an asset
  • Competent with computer use, keyboarding, and able to conduct assessments using a laptop
  • Knowledge of community agencies and resources
  • Excellent assessment, interviewing, and negotiation skills
  • Excellent verbal and written communication skills
  • Ability to work independently and co-operatively in a busy multidisciplinary situation
  • Ability to multi-task, prioritize workload, and manage time efficiently
  • Valid driver's license and access to a reliable vehicle required
  • Competencies in other languages preferred; French an asset
Hours
  • Permanent Full Time; 70 Hours Bi-weekly
  • Shifts may include days, evenings, weekends, and holidays
Compensation and Benefits

$46.75 Hourly

Successful candidates may be eligible for our benefits package, which includes health, dental, travel, long-term disability, and life insurance coverage, as well as a defined benefit pension plan.
