Lift Community Services is seeking a skilled Social Worker to join our qathet Complex Care Housing team. As a Social Worker, you will play a vital role in supporting clients with complex mental health, addiction, and chronic medical concerns to attain safe and stable housing.
Job Summary:The qathet Complex Care Housing program is a collaborative effort between Lift Community Services, Vancouver Coastal Health, and Tla'amin Nation. Our team provides a suite of services and supports to ensure individuals with complex needs achieve stable housing and thrive in their communities.
Key Responsibilities:- Conduct comprehensive social and psycho-social assessments to identify client needs and develop personalized care plans.
- Collaborate with clients, families, and interdisciplinary teams to facilitate transitions and ensure continuity of care.
- Provide individual, family, and group counseling services to support clients' emotional, social, and environmental well-being.
- Develop and maintain relationships with community resources and partners to ensure clients receive comprehensive support.
- Participate in team planning, development, evaluation, and quality improvement activities to enhance client care and services.
- Master's degree in Social Work from an accredited institution.
- Current full registration with the British Columbia College of Social Workers.
- Three years of experience working in a psychiatric setting, providing treatment services to individuals with complex mental health, addiction, and chronic medical concerns.
- Valid BC Driver's License and access to a personal vehicle for local area travel.
- Competitive hourly wage of $36.65.
