Palliative Care Nurse

We are seeking a highly skilled Palliative Care Nurse to join our interdisciplinary care team. As a key member of our team, you will provide holistic, patient-centered care that includes symptom management, coordination of care, patient and family education, emotional support, and advocacy for assigned patients.
Key Responsibilities- Work under the supervision of the Neighbourhood Home Care Specialist to deliver high-quality patient care.
- Collaborate with the interdisciplinary team to create personalized care plans based on patients' unique needs and goals of care.
- Participate in clinical huddles and case conferences with team members to ensure seamless care delivery.
- Practice according to the College of Nurses of Ontario (CNO), Regulated Health Professions Act, and Spectrum Health Care standards, policies, and procedures.
- Travel within assigned regions to patients' residences or facilities.
- Currently registered and in good standing with the College of Nurses of Ontario.
- Minimum of two years of nursing experience.
- Must have experience working with palliative care patients.
- Successful completion of Canadian Nursing Association (CNA) certification in Palliative Care Nursing or enrollment in CNA certification program in Palliative Care Nursing is considered an asset.
- Current police check suitable for working with vulnerable sectors.
- Valid Ontario driver's license and reliable vehicle with willingness to travel.
- Demonstrates problem-solving and critical thinking skills.
- Proficiency in English both written and verbal.
- Ability to work independently and prioritize workload efficiently.
- Possesses physical ability to perform daily duties.
- Demonstrates proficiency in technology use.
- Excellent patient assessment and teaching skills.
- Ability to triage client needs and escalate concerns appropriately.
- Basic proficiency in Microsoft Office software.
Our estimated salary range for this position is between $80,000 and $110,000 per year, depending on experience and qualifications. In addition to competitive compensation, we offer a comprehensive benefits package, including health insurance, dental insurance, and a generous retirement plan.
