Social Worker Specialist

Prairie Mountain Health seeks a highly skilled and compassionate Social Worker Specialist to join our team. In this role, you will provide assessment, treatment planning, counseling, service navigation, and case management services to families and individuals who are eligible for services.
Responsibilities:- Client Identification and Intake: Receive and review referral information, consult with other staff/team members, and identify clients/families who would benefit from Social Work services.
- Case Management and Resource Coordination: Assist clients in accessing fundamental needs, such as housing, financial, vocational, and support resources, and establish effective linkage to resources, agencies, and service providers.
- Psychosocial Assessment and Consultation: Implement a Social Work process to assist clients in identifying psychosocial problem areas relevant for intervention, assess family and general social supports, and provide assessment information and clinical judgment to other health service providers.
- Psychosocial Counseling: Provide brief psychosocial and adjustment counseling using recognized valid social work counseling models and methodologies.
- Crisis Intervention: Conduct an initial assessment to determine immediate psychosocial needs presented and implement use of appropriate resources to establish stability and restore client, family, and community safety.
- Qualifications: Eligible for and maintains licensure to practice as a Social Worker in the province of Manitoba, two years of related healthcare experience, proficiency with Microsoft programs, and knowledge of pertinent community resources.
- Skills: Excellent communication skills, ability to work with diverse populations, crisis intervention skills, and proficiency in providing standard Social Work services, including assessments and consultation, case management, psychosocial counseling, crisis intervention, referral, and discharge planning.
- Education: Bachelor's degree in Social Work or a related field.
- Competitive Salary: $73,000 - $87,000 per year.
