">Job Overview
">We're seeking a highly skilled Specialized Ostomy Care Professional to join our team in Downtown Toronto. In this role, you'll have the opportunity to work with a diverse range of clients, providing specialized care and support to individuals with ostomies, acute and chronic wounds, and urinary and fecal continence concerns.
">Key Responsibilities
">- Provide holistic assessment and management to clients with complex needs, working collaboratively with interprofessional teams to develop personalized care plans.">
- Formulate treatment recommendations and develop plans of care based on comprehensive assessments, involving the Wound Ostomy Resource Nurse (WORN) and/or K2P Wound Care Resource Nurses (K2P WCRN as appropriate)">
- Evaluate the effectiveness of care plans, identifying areas for improvement and implementing quality monitoring processes to optimize client outcomes.">
- Develop and deliver education and mentorship programs to build wound, ostomy, and continence knowledge and capacity within SE Health.">
- Consult with care providers to ensure high-quality care and provide expert advice on wound, ostomy, and continence management.">
- Collaborate with interprofessional teams to reduce duplication of services, client anxiety, and costs, promoting efficient and effective care delivery.">
Requirements
">To be successful in this role, you'll need:
">- A completed Nurses Specialized in Wound, Ostomy & Continence Canada (NSWOCC) WOC-EP program (international certification will be considered if approved by NSWOCC).">
- Flexibility and a commitment to advancing wound, ostomy, and continence care at SE Health.">
- A willingness to participate in further education programs/courses to remain current.">
- The ability to use computer word processing, PowerPoint, email, and source information on the internet.">
- A valid driver's license and a willingness to travel.">
- A comprehensive knowledge of community health care is preferred.">
What We Offer
">As a Specialized Ostomy Care Professional with SE Health, you'll enjoy a competitive salary of $95,000 - $110,000 per year, depending on experience, plus benefits, pension, and opportunities for professional growth and development.
