About the Role:

We are seeking a highly skilled Wound Ostomy and Continence Care Specialist to join our team at SE Health in Downtown Toronto. As a member of our interprofessional team, you will provide specialized holistic assessment and management to meet the needs of individuals with ostomies, acute and chronic wounds, and urinary and fecal continence concerns.

Key Responsibilities:

  • Perform detailed holistic assessments of clients with wound, ostomy and/or continence related concerns
  • Formulate treatment recommendations in collaboration with the interprofessional team, and develop a plan of care based on the holistic assessment for clients who require wound, ostomy, and/or continence care
  • Provide education and mentorship experiences to build wound, ostomy and continence knowledge and capacity within SE Health
  • Consult with care providers within the organization to promote best practices in wound, ostomy and continence care
  • Collaborate with interprofessional teams across the continuum of care to reduce duplication of services, client anxiety, and cost

Requirements:

  • Nurses Specialized in Wound, Ostomy & Continence Canada (NSWOCC) WOC-EP program or international certification approved by NSWOCC
  • Current and active membership in NSWOC Canada, preferred
  • RN with current certificate of competence with the College of Nurses of Ontario
  • Flexibility and commitment to advancing wound, ostomy, and continence care at SE Health
