Overview

CarePartners is seeking a skilled Registered Nurse or Registered Practical Nurse to join our team in providing exceptional nursing services to patients in their homes and communities. As a member of our interdisciplinary care team, you will have the opportunity to develop and maintain effective care plans, work with patients to provide personalized nursing services, and communicate with patients, family members, and other healthcare professionals.

Key Responsibilities

  • Develop and maintain care plans for patients in community health services
  • Provide nursing services to patients in their homes, clinics, and other settings
  • Communicate with patients, family members, and other healthcare professionals to ensure seamless care
  • Work collaboratively with the interdisciplinary care team to achieve patient outcomes

Requirements

  • Registered Nurse (RN) or Registered Practical Nurse (RPN) degree or diploma
  • Current College of Nurses of Ontario (CNO) registration in good standing
  • Exceptional interpersonal skills and a strong desire to make a difference in the lives of our patients
  • A valid driver's license and vehicle as regular travel is required
  • Excellent English verbal and written communication skills
  • CPR Certification
  • Current immunization record including 2 step TB test
  • To protect our patients, we require a current vulnerable sector check and a clear background check
