Key Responsibilities- Prepare reports and case histories to document client progress
- Assess clients' needs and eligibility for specific services
- Develop and implement rehabilitation programs to support clients' reintegration into the community
- Plan and implement life skills workshops to enhance clients' independence
- Conduct follow-up assessments to evaluate clients' progress
- Develop and maintain relationships with other social services agencies and healthcare providers
- Assist clients with special needs and accompany them to appointments and social outings
Requirements:
- College/CEGEP education
- 2 years to less than 3 years of experience in a related field
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
What We Offer:
