Job Description

The Case Worker in Ontario Structured Psychotherapy (OSP) plays a vital role in the coordination of OSP. This includes triaging referrals, communicating with referral sources, and presenting complete referrals to intake clinicians for acceptance into the program.

As a member of the OSP team, you will be responsible for:

  • Making support referrals
  • Triaging referrals received by Ontario Shores
  • Facilitating admission to Ontario Shores for Ontario Structured Psychotherapy
  • Facilitating referrals for community support/intervention
  • Providing information and system navigation to individuals, families, and other care providers

You will also be expected to recognize and report patient, staff, and environment safety issues, identify and implement preventative and corrective actions within the scope of this role.
