Job Title: Social Worker, Red Blood Cell Disorders Clinic

University Health Network (UHN) is seeking a highly skilled Social Worker to join our Blood Disorders Program. As a Social Worker, you will play a vital role in supporting adult outpatients in our Red Blood Cell Disorders (RBCD) clinic.

Key Responsibilities:
  • Collaborate with a multidisciplinary team to provide comprehensive support to patients living with hemoglobinopathies, including Thalassemia and Sickle Cell Disease.
  • Develop and implement individualized intervention plans to address the unique needs of each patient, taking into account their cultural, social, and economic circumstances.
  • Provide counseling and case management services to patients and their families, focusing on empowerment, strengths-based approaches, and trauma-informed care.
  • Work closely with community agencies and healthcare providers to ensure seamless care coordination and access to resources.
  • Participate in research and quality improvement initiatives to enhance patient outcomes and experiences.
Requirements:
  • Master's degree in Social Work from a recognized institution.
  • Registration with the Ontario College of Social Workers and Social Service Workers.
  • Minimum 2-3 years of experience in social work, preferably in a healthcare setting.
  • Strong understanding of Anti-Oppression, Anti-racism, Human Rights, and Social Justice principles.
  • Excellent communication, interpersonal, and advocacy skills.
  • Ability to work effectively in a fast-paced environment and prioritize multiple tasks.
