Job Summary

We are seeking a skilled Social Worker to join our Primary Care Network team at Island Health. As a key member of our team, you will work closely with primary care providers, other disciplines, and community partners to deliver high-quality patient care.

Key Responsibilities
  • Provide counseling services, including crisis, loss, and grief counseling, to patients and their families.
  • Determine and deal with changing and emerging situations with patients in a flexible and adaptable manner.
  • Foster effective relationships with primary care providers, other disciplines, and community partners to promote a team approach to patient care.
  • Utilize knowledge of primary health care principles, community development concepts, and the strategic direction of Island Health to inform practice.
  • Develop and maintain a working knowledge of facilitation, mediation, and conflict resolution skills and techniques.
  • Provide case management, clinical support, and consultation services to patients and their families.
  • Prioritize and organize workload to ensure efficient and effective delivery of services.
Requirements
  • Bachelor's Degree in Social Work from an accredited university.
  • Current full registration with the British Columbia College of Social Workers.
  • Two years' recent related experience in a healthcare setting working with medically complex clients with concurrent disorders, or a combination of education, training, and experience.
  • Valid BC Driver's License.
