Qualifications and Requirements

We are seeking a skilled Social Worker to join our Primary Care Network team. The ideal candidate will possess a Bachelor's Degree in Social Work from an accredited university and be registered with the British Columbia College of Social Workers.

With a minimum of two years' experience in a healthcare setting, preferably working with medically complex clients with concurrent disorders, you will have a comprehensive understanding of chronic health challenges, geriatrics, community health principles, human development, behavior, and family/group dynamics.

Additionally, you will have a working knowledge of applicable legislation and available programs and services, their interrelationships, and their function in delivering care in the community.

A valid BC Driver's License is also required.

Key Skills and Abilities
  • Effective counseling skills, including crisis, loss, and grief counseling.
  • Ability to adapt to changing situations and prioritize tasks effectively.
  • Strong communication and interpersonal skills, with the ability to build effective relationships with primary care providers, other disciplines, and community partners.
  • Basic computer skills and knowledge of primary health care principles, community development concepts, and the strategic direction of the Vancouver Island Health Authority.
  • Facilitation, mediation, and conflict resolution skills, as well as analytical and problem-solving skills.
  • Ability to work independently and as part of a multidisciplinary team, with a proactive and facilitative approach to patient care.
  • Excellent communication and interpersonal skills, with the ability to act as a clinical resource and educator.
