About the Role: We are seeking an experienced registered nurse, physiotherapist, occupational therapist, or social worker to join our team as a Care Coordinator. In this position, you will work collaboratively with patients and their families to assess care needs, determine eligibility for services, and develop individual care plans. You will also link patients with service providers, coordinate and monitor care plan delivery, and establish a helping relationship with patients and their families.

Key Responsibilities:
  1. Assess patient care needs and determine eligibility for services
  2. Provide access to community resources
  3. Engage with patients, caregivers, and healthcare practitioners to deliver patient-centered care
  4. Develop individual care plans
  5. Link patients with service providers
  6. Coordinate and monitor care plan delivery

Requirements: To be successful in this role, you must have membership in good standing with the applicable regulatory body, recent experience in community health or a related field, knowledge of the healthcare delivery system and community resources, excellent interpersonal, communication, assessment, problem-solving, and decision-making skills, effective time management, prioritization, and organizational skills, and the ability to work independently and co-operatively in a busy multidisciplinary environment.
