About the Role:
As a Nurse Practitioner - UroOncology Care Coordinator at Sinai Health, you will play a vital role in the management of UroOncology patients within the Department of Urology/MKUWC at Mount Sinai Hospital.
Responsibilities include:
- Collaborating with attending physicians to assist with patient clinics, chart preparation, and nursing duties such as catheterization, catheter care, pre and post-op surgery teaching, and other duties as directed.
- Coordinating, delivering, supervising, and evaluating patient care.
- Systematically assessing patients' physical and psychosocial needs.
- Assuming responsibility for health education and counselling of patients.
- Assisting patients and their families in identifying resources leading to the maximum state of health.
- Planning, organizing, and determining priorities in the giving of patient care.
- Active involvement in the overall coordination of patient care.
- Reporting and recording accurately and appropriately, using electronic charting.
- Undertaking triage responsibilities, including telephone triage.
- Assessing, planning, implementing, and evaluating a broad range of patient care activities related to health promotion and chronic disease management.
Requirements:
Current certificate of registration with the College of Nurses' of Ontario.
Current BCLS.
BScN (preferred).
Minimum of three (3) years recent urology, urologic, and oncology patient nursing experience.
Excellent teaching skills experience that incorporates adult learning principles.
Flexible with superior organization skills and the ability to work effectively to prioritize multiple, competing tasks in a fast-paced and dynamic environment.
Ability to exercise sound judgement and discretion, make decisions, and take initiative while working with confidential patient information and responding to patient inquiries.
Superior work ethic, professionalism, and reliability; excellent interpersonal and relationship building skills; ability to effectively convey factual information and detailed explanations.
Sound knowledge base in urology, urologic procedures, surgeries, and patient care.
Commitment to professional practice and personal development.
Commitment to shared leadership and team-based decision making.
Commitment to patient and family centred care.
Demonstrated satisfactory work performance and attendance history.
