Job Summary

VON Canada is seeking a Clinical Care Specialist to join our team in Nova Scotia. As a Clinical Care Specialist, you will provide medical care to children, youth, adults, and seniors in their homes and other settings in communities across Nova Scotia.

Key Responsibilities

  • Provide medical care to patients in their homes and other settings
  • Manage diabetes, dialysis, IV medications, and other medical conditions
  • Work collaboratively with healthcare teams to provide comprehensive care
  • Develop and implement individualized care plans
  • Monitor patient progress and adjust care plans as needed

Requirements

  • Current registration as a Registered Nurse in the province of practice
  • Experience in community health an asset
  • Valid driver's license and access to a reliable vehicle
