Clinical Navigation Specialist

We are seeking a skilled Clinical Navigation Specialist to join our Rehabilitation Clinic team at IWK Health Centre in Halifax, Nova Scotia. This role plays a crucial part in providing exceptional patient care by facilitating intakes, offering support and information to families, and collaborating with the inter-professional healthcare team.
Job SummaryThe Clinical Navigation Specialist is responsible for completing intakes with patients and families, gathering clinical information, and applying it to decision-making models to provide appropriate next-step follow-up appointments or alternatives. They will also support referrals to the most suitable care pathways based on a clinical algorithm designed by the care team. Strong knowledge of available resources within the IWK Health Centre and community is essential for this role.
Responsibilities- Completing intakes (virtually) with families of the Rehabilitation clinic and supporting referrals to the most appropriate care pathways
- Providing information and support to patients and families at entry and transfer points in the Rehabilitation Clinic
- Collaborating with the inter-professional healthcare team to ensure seamless patient care
- Maintaining up-to-date knowledge of IWK and community resources to effectively navigate systems
- Using databases to collect patient information and retrieve reports related to patient demographics and wait times
- Bachelor of Social Work required; other health care professional degrees with experience in facilitating intakes may be considered
- Registration or eligibility for registration with the Nova Scotia College of Social Workers (NSCSW) is required
- Minimum two years of related clinical experience is mandatory
- Three to five years of experience in intake and/or client navigation is preferred
- Experience in pediatric rehabilitation is highly desirable
- Current successful completion of a Basic Life Support (BLS) Program within the last 12 months is required
