Job Summary

The Community Support Worker III plays a vital role in the Menatl Health and Addictions Services team at Prairie Mountain Health. This position requires a client-centered approach to help individuals maintain wellness in the community, supporting their goals within the principles of quality care.

Key Responsibilities

  • Provides a holistic approach to strengthen client skills, promoting independence, self-confidence, and dignity.
  • Assists clients to be active participants in the management of their own environment, developing a broader network of support.
  • Conducts self in a professional, tactful, and courteous manner when interacting with staff, clients, families, and volunteers.
  • Schedules activities and programming to meet identified client needs, in consultation with the supervisor.
  • Assists in the orientation of new staff and students as requested.

Individualized Supports to Clients and Families

  • Works with the supervisor and other team members to develop an appropriate plan of care, examining client strengths and needs.
  • Conducts appointments and home visits in a professional and ethical manner, connecting with clients to develop a therapeutic working relationship.
  • Assists clients with individualized plans of care, linking them with existing community resources.

Requirements

This position requires a professional, tactful, and courteous demeanor when interacting with staff, clients, families, and volunteers. The successful candidate will have excellent communication and interpersonal skills, with the ability to work collaboratively as part of a team.
