Life Skills Support Worker

We are seeking a highly motivated and compassionate Life Skills Support Worker to join our Primary Care team. As a member of our interprofessional team, you will work under the direction of the Team Leader to provide person-centered care to individuals with complex health issues. Your primary goal will be to maximize independent living by applying best practices and providing services outlined in individual care plans.
About the Role
As a Life Skills Support Worker, you will be responsible for providing support and assistance to individuals with daily living tasks, such as personal care, meal preparation, and household management. You will also work collaboratively with other healthcare professionals to develop and implement individualized care plans that promote independence and well-being. Your excellent communication and interpersonal skills will enable you to build strong relationships with clients, families, and colleagues, ensuring a seamless and effective care experience.

Requirements
Diploma from a recognized Social Services Support Worker educational program, or Residential Care Aide program, two (2) years' recent, related experience working with people with complex health issues, or an equivalent combination of education, training and experience.
Or Grade 12 plus a Community Mental Health Worker Certificate and Addictions Studies Certificate plus three (3) years recent related experience working with adults with mental illness and/or addictions diagnosis; or an equivalent combination of education, training and experience.
Valid . driver's license.
Valid First Aid Certificate, CPR and Food Safe.

Conflict resolution and crisis intervention skills.
Advocacy skills.
Basic computer skills including windows, word, internet and e-mail
Ability to teach and work effectively in both groups setting and one-to-one.
