Community Health Worker II Position

As a Community Health Worker II, you will be part of an interprofessional healthcare team providing home support services to clients in accordance with a pre-established care plan. Your key responsibilities will include assisting with activities of daily living, promoting maximum independence, and ensuring the well-being of individuals and families.
About the Role
Under the direction of the Team Lead or designated clinical leader, you will work with clients to develop and implement personalized care plans. You will also perform delegated tasks for which delegation of tasks training has been completed. A key component of this job is to promote independent living while in the community.

Requirements
To be successful in this role, you will need to have a graduation from a recognized Home Support Attendant/Resident Care Aid Program. You will also need to be registered with the BC Care Aid & Community Health Worker Registry and have a valid Class 5 BC Drivers' License. Certificates in CPR, First Aid, and Food Safe are also required. Use of a personal vehicle is necessary for this role.
Skills and Abilities
As a Community Health Worker II, you will need to have the ability to establish and maintain rapport with clients. You will also need to be able to observe and recognize changes in clients and communicate verbally and in writing with clients, family, and care team. You will need to be able to work independently and in cooperation with others, organize and prioritize work, and operate related equipment. Home management skills are also essential for this role.
