Job Summary

WoodGreen is seeking a skilled Case Manager to join our Supportive Housing Program. As a Case Manager, you will provide comprehensive case management services to individuals in the program, including assessment, personal goal setting, and referrals to community resources. You will work closely with staff and clients to develop and implement individualized plans, and provide ongoing support and guidance to help clients achieve their goals.

Key Responsibilities
  • Provide case management services to individuals in the program, including assessment, personal goal setting, and referrals to community resources.
  • Develop and implement individualized plans with clients, and provide ongoing support and guidance to help them achieve their goals.
  • Work closely with staff and clients to identify and address mental health issues, and connect clients with primary care and other needed resources.
  • Assist tenants with daily living tasks, such as grocery shopping and meal preparation, and provide support with life skills development.
  • Link clients to community resources and facilitate referrals to other support services as needed.
  • Monitor client progress and adjust plans as needed to ensure successful outcomes.

Requirements
  • Bachelor's degree in Social Work or a related field.
  • 3+ years of experience in mental health and housing support.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Knowledge of community resources and support services.
