We are seeking a skilled and compassionate Licensed Practical Nurse to join our Enhanced Care Centre team in Bridgewater, Nova Scotia.
Key Responsibilities- Perform initial and regular health assessments to maintain patient quality of life.
- Support development of individualized care plans that focus on the patient's functional abilities and support network.
- Assess, plan, implement, and evaluate nursing care of patients.
- Ensure compliance with Philosophy of Service and patient care plan to ensure optimal health and wellness for each patient.
- Manage patient medication in accordance with legislative requirements.
- Follow the process in place for safekeeping of medications.
- Administer and record prescribed medications in accordance with facility policy.
- Provide consultation regarding patient issues and standards of care to interdisciplinary team.
- Communicate with all other departmental teams to increase awareness of patient health changes to ensure effective service.
- Ensure all completed care tasks are documented appropriately using identified technology and tools.
We are looking for a candidate with a Diploma in Practical Nursing and current registration with the Nova Scotia College of Nursing. You should have comprehensive knowledge of nursing and health care practices, excellent communication and interpersonal skills, and availability to work a schedule of flexible hours and all shifts inclusive of days, evenings, nights, weekends, and holidays.
