Job Summary

The Home Care Nurse will provide professional nursing care to residents, including medication administration, mentoring and coaching unregulated staff, and developing care plans under the direction of the Director of Care or designate.

Responsibilities:
  • Develop resident-centered care plans that prioritize physical, psychological, and social well-being.
  • Direct and assist care aides in providing high-standard resident care, adhering to established procedures.
  • Administer, monitor, and record medication administration, educate residents as needed, and maintain accurate records.
  • Create and manage nursing care plans, participate in evaluating current and future medical needs, and communicate changes to the Health & Wellness Manager or designate and/or the resident's physician.
  • Evaluate the effectiveness of nursing care treatments, inform the Health & Wellness Manager or designate and/or the resident's physician of significant changes in resident condition, and work with residents to meet care requirements.
  • Provide personal care assistance, nursing procedures, mentorship, and coaching of Health Care Aides, ensuring ongoing competence in caring for aging adults.
  • Communicate effectively with residents, doctors, and managers to ensure seamless care delivery, maintaining awareness of provincial continuing care health service standards, legislation, codes, regulations, and employment and safety statutes.
Requirements:
  • Nursing diploma completion.
  • Registered with provincial nursing regulatory body and in good standing.
  • Experience in residential care settings preferred.
  • Demonstrate infection control procedures, exceptional interpersonal skills, superior organizational skills, and flexibility to work varied shifts.
  • Ability to stand for extended periods, bend, squat, kneel, and lift 25 pounds, while displaying compassion, education, proficiency, and experience.
Benefits:
  • Estimated salary: $60,000 - $80,000 per annum, depending on experience.
  • CPR and First Aid certification required.
