Nurse Practitioner

Job Description:
As a Nurse Practitioner, you will provide high-quality patient care in community-based settings. This role involves assessing patients' physical and social environments to develop comprehensive care plans. You will work collaboratively with interdisciplinary teams to ensure the best possible outcomes for patients.
Key Responsibilities:
- Conduct thorough assessments of patients' health status and develop individualized care plans
- Provide education and support to patients and their families to promote healthy behaviors and disease prevention
- Collaborate with healthcare teams to coordinate care and ensure continuity of services
- Maintain accurate and up-to-date records of patient interactions and progress
Requirements:
- Completion of an accredited nursing education program and current registration with the College of Registered Nurses of Alberta (CRNA)
- Minimum 1 year of community-based nursing experience, including geriatric experience obtained within the last three years
- Ability to work independently and as part of a team, with strong communication and problem-solving skills
- Knowledge of computer applications, including Meditech, Connect Care, and Microsoft Office
Salary: $35.87-$43.82 per hour, with the possibility of adjustment upon being entered into the register of Registered Nurses.
Working Conditions:
- This role requires working in various environments, including community/client homes, Independent Living Sites, and Designated Supported Living facilities
- Must be able to tolerate long periods of sitting or standing and repetitive movements
- Lifting and carrying up to 10-15 kg may be required on occasion
