About the Job

The Licensed Practical Nurse - Transition Liaison plays a vital role in ensuring seamless patient care transitions between Interior Health facilities and healthcare services across the continuum of care.

As a key member of the multidisciplinary team, the LPN takes a lead role in planning, implementing, evaluating, and coordinating patient access and care transition activities. This includes working collaboratively with healthcare professionals to assess patient needs and eligibility for appropriate services and resources.

The ideal candidate will possess strong communication and interpersonal skills, with the ability to work effectively in a fast-paced environment. A strong understanding of the British Columbia College of Nurses and Midwives (BCCNM) standards of practice and a commitment to patient-centered care are essential.

Responsibilities
  • Plan, implement, evaluate, and coordinate patient access and care transition activities
  • Work collaboratively with multidisciplinary teams to assess patient needs and eligibility for services and resources
  • Act as a resource to patients, families, and healthcare teams on patient care and transition planning
  • Operate within the full scope of practice as outlined by the BCCNM and organizational policies
Requirements
  • Graduation from a recognized program for Practical Nurses
  • One year of recent related clinical experience, including a combination of community and acute care nursing experience
  • Current practicing registration with the British Columbia College of Nurses and Midwives (BCCNM)
