We are seeking a highly skilled Psychosocial Care Specialist to join our team at Fraser Health Authority. This is an exciting opportunity for a motivated and compassionate individual to make a meaningful difference in the lives of our patients, families, and communities.
About the RoleAs a Psychosocial Care Specialist, you will work closely with interdisciplinary teams to provide comprehensive psychosocial assessments and therapeutic interventions to clients in need. Your expertise will be instrumental in promoting improved mental health outcomes, enhancing quality of life, and facilitating effective care planning.
Key Responsibilities- Conduct thorough psychosocial assessments to identify client needs and develop personalized treatment plans
- Provide evidence-based therapeutic interventions, including counseling, education, and support
- Collaborate with healthcare professionals and community agencies to ensure seamless care coordination
- Maintain accurate and up-to-date records of client interactions and progress
To be successful in this role, you will require:
- A Bachelor's Degree in Social Work from an approved institution, plus one year of recent clinical experience
- Current full registration with the BC College of Social Workers (BCCSW)
- Exceptional communication and interpersonal skills, with the ability to work effectively with diverse populations
- Strong knowledge of psychosocial theories, therapeutic modalities, and current best practices in social work
