Job Summary

We are seeking a highly skilled and experienced Social Services Coordinator to join our team at Kensington Health Centre. As a key member of our behavioral support team, you will play a vital role in supporting our residents who require complex behavioral support.

Key Responsibilities
  • Collaborate with the Behavioral Support Lead to develop and implement effective behavioral support plans for residents.
  • Work closely with the multi-disciplinary team to identify and address psychosocial behavior and life adjustment challenges.
  • Conduct thorough assessments and evaluations to determine the needs of residents and their families.
  • Develop and maintain strong relationships with community agencies and healthcare providers to ensure seamless care coordination.
  • Provide counseling and support to residents and their families to help them adjust to living in our facility.
  • Participate in interdisciplinary team conferences and contribute to the development of comprehensive treatment plans.
  • Coordinate family council meetings and act as a liaison between family council and the facility.
  • Assist with the completion of 21-day assessments for new residents and ensure that all necessary paperwork is completed.
  • Ensure that all financial arrangements are made for new residents, including review and signing of admission contracts and uninsured services forms.
  • Act as a resource for resident advocacy and provide support with financial matters and application forms.
Requirements
  • Minimum three (3) years' experience in Long Term Care with specialized knowledge of Gerontology, Alzheimer Dementia, Behavioral Disorders, Complex and Palliative Care.
  • Minimum 2 years' experience working with older adults who experience responsive behaviors.
  • Minimum 2 years' experience working with older adults who are severely cognitively impaired.
  • Minimum Masters of Social Work (MSW) degree and in good standing certificate with OCSWSSW.
Preferred Qualifications
  • Experience working in a multi-disciplinary team environment.
  • Strong assessment and evaluation skills, with the ability to plan, organize and implement both proactively and responsively.
  • Excellent communication and interpersonal skills, with the ability to work effectively with residents, families, and healthcare providers.
  • Competency with Point Click Care and MS Suite (Word, PowerPoint and Excel).
