Forensic Community Specialist

BC Mental Health & Substance Use Services is seeking a highly skilled and compassionate Forensic Community Specialist to join our team in Prince George, BC.
Job Summary:We are looking for a Registered Nurse (RN) or Registered Psychiatric Nurse (RPN) with experience in working with individuals with severe and persistent psychiatric illness. As a Forensic Community Specialist, you will conduct initial intake and ongoing biopsychosocial nursing psychiatric assessments, develop and implement Integrated Treatment Plans, and provide community case coordination and advocacy services.
Key Responsibilities:- Conduct comprehensive assessments and develop treatment plans in collaboration with clients, families, and healthcare teams.
- Provide community case coordination and advocacy services to ensure continuity of care and access to community resources.
- Maintain accurate and concise documentation of client information and treatment plans.
- Collaborate with healthcare teams, community agencies, and other stakeholders to ensure comprehensive care and support.
- Graduation from an approved School of Nursing with current practicing registration as a Registered Nurse or Registered Psychiatric Nurse with the British Columbia College of Nurses & Midwives (BCCNM).
- Two (2) years recent, related experience working with individuals in the community with severe and persistent psychiatric illness.
- Valid BC Drivers License.
