Job Summary:

We are seeking a motivated and organized Licensed Practical Nurse (LPN) to take on a Team Leader role within our Home Support Services team. As a Team Leader, you will be responsible for supervising LPNs, Community Health Workers, and other unregulated team members, overseeing the implementation of client care plans, and providing training and evaluation of care provided by staff.

About the Role:

This is a great opportunity for an experienced LPN to continue to develop and enhance their first-line supervisory skill-set. The ideal applicant will be confident and experienced in providing direction and assignments to staff, organizing workflow, and ensuring that safe and individualized client-centered care is always delivered.

Responsibilities:

- Coordinate Home Support Services by supervising LPNs, CHWs, and other unregulated team members
- Oversee the implementation of client care plans
- Provide training and evaluation of care provided by staff
- Advocate for changes that improve client care

Requirements:

- Current LPN registration with the British Columbia College of Nurses and Midwives
- Experience in supervisory or leadership roles
- Strong communication and interpersonal skills
