Job Summary

The Registered Nurse (RN) role models clinical excellence and the core competencies of the practice standards of the College of Nurses of Ontario (CNO), as well as the policies and procedures of Southlake Regional Health Centre (Southlake). The RN works collaboratively as a member of the health care team, providing direct patient care to patients based on the three-factor framework as outlined in the College of Nurses of Ontario standard, RN and RPN Practice: The Client, Nurse and the Environment.

Primary Responsibilities
  • Collects all relevant clinical information in preparation for in-home/community assessment
  • Provides comprehensive in-home assessment to seniors, considering their physical, emotional, perceptual and cognitive needs
  • Synthesizes the clinical information in order to develop recommendations
  • Comprehensively documents assessment findings and recommendations
  • Facilitates linkages between the patient and suitable community resources
  • Participates in weekly interprofessional team rounds
Qualifications
  • Bachelor of Science in Nursing (BscN) preferred
  • Registered and in good standing with the College of Nurses of Ontario (CNO) as a Registered Nurse
  • BCLS current certification required
  • Willingness to obtain certification to administer cognitive testing i.e. (Montreal Cognitive Assessment – MoCA); to be successfully completed within three (3) months of hire and supported financially, if required
  • Three to Five years of current/recent geriatric experience (acute medicine and community) required
  • Strong knowledge of community-based services available to seniors required
  • Valid driver's license and access to own vehicle required for community visits
  • Excellence in delivering evidence-based, patient/family inspired care
  • Proven ability to advocate for clients and families
  • Demonstrates skill and interest in working with the geriatric population and/or people who are challenged by physical, emotional, perceptual and cognitive difficulties
  • Working knowledge of the Regulated Health Professions Act, Mental Health Act, Health Care Consent Act and Substitute Decisions Act
  • Possess an attitude of respect and inclusivity towards clients with varied backgrounds, abilities and educational status
  • Commitment to functioning as part of a dynamic inter-professional team
  • Demonstrates excellence in communication, both written and verbal
  • Demonstrates superior facilitation, problem solving skills and conflict resolution
  • Demonstrates initiative and individual leadership skills
