Job Summary

The Advanced Geriatric Care Specialist role models clinical excellence and core competencies, providing direct patient care to elderly patients demonstrating age-related complexities and/or frailty in their home environment.

Key Responsibilities:
  • Collects relevant clinical information for in-home/community assessment
  • Provides comprehensive in-home assessments considering physical, emotional, perceptual, and cognitive needs
  • Synthesizes clinical information to develop recommendations
  • Documents assessment findings and recommendations
  • Facilitates linkages between patients and suitable community resources
Qualifications:
  • Bachelor of Science in Nursing (BScN) preferred
  • Registered and in good standing with the College of Nurses of Ontario (CNO) as a Registered Nurse
  • BCLS current certification required
  • Willingness to obtain certification to administer cognitive testing (e.g., Montreal Cognitive Assessment – MoCA)
  • Three to Five years of current/recent geriatric experience (acute medicine and community) required
  • Strong knowledge of community-based services available to seniors required
  • Valid driver's license and access to own vehicle required for community visits
Salary and Benefits:

The estimated salary for this position is $80,000 - $110,000 per year, depending on qualifications and experience. The package includes comprehensive health benefits, pension plan, and opportunities for professional growth and development.
