Fraser Health is seeking a skilled and compassionate Indigenous Health Liaison Nurse to join our team. As a key member of our Indigenous Health Liaison Team, you will play a vital role in planning, organizing, and evaluating healthcare needs and services for Indigenous clients in community and hospital settings.
Key Responsibilities:- Act as a liaison between Indigenous clients, families, service providers, and community agencies to enhance service delivery.
- Conduct client assessments and screenings to evaluate emotional, physical, spiritual, and mental status.
- Develop and implement care plans in collaboration with clients, families, and service providers.
- Provide health education and health promotion services to individuals, families, and Indigenous communities.
- Participate in multidisciplinary teams and committees to integrate Indigenous Health with hospital and community programs.
- Current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
- Graduation from an approved school of nursing including public health nursing component.
- Two years' recent, relevant nursing experience.
- Knowledge of Indigenous health approaches, protocol, and cultural issues.
- A dynamic and supportive work environment.
