Job Description

As an Advanced Healthcare Provider, you will play a vital role in delivering high-quality patient care services in various settings. Your primary responsibilities will include conducting thorough assessments, diagnosing and managing acute and chronic illnesses, and developing comprehensive care plans to ensure optimal patient outcomes.

  • Key Responsibilities:
  • Performing physical examinations and health assessments to evaluate patient needs and conditions.
  • Diagnosing and managing acute and chronic illnesses, developing treatment plans that include medication management and lifestyle recommendations.
  • Collaborating with healthcare teams, including physicians and specialists, to ensure cohesive patient care and follow-up.
  • Educating patients and their families on health management, treatment options, and preventive care strategies to promote wellness.
  • Maintaining accurate and timely records of patient encounters, treatment plans, and outcomes in electronic health records (EHR).
  • Staying informed about the latest advancements in healthcare practices and contributing to quality improvement initiatives within the organization.
  • Serving as a patient advocate, providing support and guidance throughout the healthcare process while respecting patient autonomy and confidentiality.
Requirements
  • Education: Masters degree in Nursing (MSN) from an accredited program.
  • Licensure: Valid state NP license and current CPR certification.
  • Experience: Previous experience as a Nurse Practitioner preferred; specialized training in areas such as family practice, pediatrics, or geriatrics is a plus.
  • Skills: Strong clinical skills, critical thinking abilities, and excellent interpersonal communication skills.
  • Personality: Demonstrated ability to provide compassionate, patient-centered care while ensuring confidentiality.
