Job Title: Pharmacist Consultant

Join our team of dedicated healthcare professionals at Lawtons, a leading pharmacy chain in Canada. As a Pharmacist Consultant, you will play a vital role in providing exceptional patient-centered care and exceeding patients' expectations.

About the Role:

We are seeking a highly skilled and compassionate Pharmacist to join our team. As a Pharmacist Consultant, you will be responsible for:

  • Providing clinical services focused on continuing care contractual obligations, including disease state management, medication reviews, and full scope of practice.
  • Participating in professional programs and collaborating with colleagues to provide excellence in health & wellness services to all patients.
  • Initiating care plans, following up, and completing proper documentation.
  • Participating on facility committees, completing biannual medication reviews, and ensuring prescription safety.
  • Providing quarterly reports to facility administration and reviewing all medication errors.
