Pharmacist Role at Lawtons

We are seeking a skilled Pharmacist Consultant to join our team at Lawtons. As a key member of our pharmacy team, you will play a vital role in providing exceptional patient-centered care and exceeding patient expectations.

Key Responsibilities:
  • Deliver high-quality patient care by demonstrating warmth, empathy, and compassion while achieving excellence in patient-centered care, customer service, and workflow efficiencies.
  • Provide clinical services focused on continuing care contractual obligations, including disease state management, medication reviews, and reporting.
  • Collaborate with colleagues to provide excellence in health & wellness services to all patients.
  • Initiate care plans, follow up, and complete proper documentation.
  • Identify actual and potential drug-related problems through completion of medication reviews.
  • Provide education to nursing staff.
  • Participate on facility committees, complete biannual medication reviews, and ensure prescription safety.
  • Provide quarterly reports to facility administration and review medication errors.
  • Maintain current knowledge of pharmacy practice, regulatory requirements, and industry trends/developments.
Requirements:
  • Bachelor of Science in Pharmacy from a recognized institution.
  • Licensed Pharmacist, in good standing, in the province of responsibility.
  • Injection and First Aid Certification.
  • Additional certifications (i.e. Certified Geriatric Pharmacist, Advance Prescribing).
  • Kroll software experience is preferred.

Work Schedule:
  • 10 hour shift.
  • 12 hour shift.
  • 8 hour shift.
  • Monday to Friday.
  • Weekends as needed.
