Pharmacist Consultant Job Description

At Lawtons, we're committed to delivering exceptional patient-centered care and exceeding patients' expectations. As a Pharmacist Consultant, you'll play a key role in achieving excellence in patient care, customer service, and workflow efficiencies.

Key Responsibilities:
  • Provide clinical services focused on continuing care contractual obligations, including disease state management, medication reviews, and full scope of practice.
  • Participate in professional programs and collaborate with colleagues to provide excellence in health & wellness services to all patients.
  • Initiate care plans, follow up, and complete proper documentation.
  • Participate on facility committees, complete biannual medication reviews, and ensure prescription safety.
  • Provide quarterly reports to facility administration and review all medication errors.
Requirements:
  • Bachelor of Science in Pharmacy from a recognized institution.
  • Licensed Pharmacist, in good standing, in the province of responsibility.
  • Kroll software experience is preferred.
