About the Role

We are seeking a skilled Registered Nurse to join our team as a Nurse Case Manager. In this role, you will work closely with physicians and other healthcare professionals to provide comprehensive patient support and care.

Key Responsibilities

  • Provide patient education and support on medication management and treatment plans
  • Collaborate with healthcare teams to develop and implement individualized care plans
  • Communicate effectively with patients, families, and healthcare providers to ensure seamless care coordination
  • Monitor patient progress and adjust care plans as needed
  • Participate in quality improvement initiatives to enhance patient outcomes and experiences

Requirements

  • Current registration as a Registered Nurse with the provincial regulating body
  • Experience in immunology and patient support is an asset
  • Strong communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Proficiency in Microsoft Office and electronic medical records
