Job Summary

We are seeking a highly skilled and compassionate Nurse Case Manager to join our team at CB Canada. As a key member of our healthcare team, you will play a vital role in providing exceptional patient care and support to our clients.

Key Responsibilities
  • Provide Medical Support: Offer medical guidance and support to patients and healthcare providers, including education on specific diseases and therapies, and disease management.
  • Telephone Support: Provide telephone support, including maintenance calls and adherence support, as needed.
  • Database Management: Manage databases and conduct ongoing audits of files and information captured.
  • Case Management Protocols: Administer and adhere to case-management protocols, including clinical monitoring, data collection, on-going patient follow-up, compliance, therapy disruption/discontinuation monitoring, adverse event reporting, and liaison with primary healthcare providers.
  • Training and Development: Prepare for and attend extensive, specialized training sessions for specific therapies provided by the pharmaceutical team, as required.
  • Reimbursement and Funding: Work directly with the reimbursement specialist and the physician to secure funding for patient therapies, and assist with alternative funding searches when needed.
  • Meetings and Conferences: Attend pharmaceutical sales meetings, patient meetings, and conferences as required, to educate patients and providers regarding disease/product information and case management services.
  • Partnerships and Collaborations: Establish and maintain partnerships with pharmaceutical teams, patient associations, and advocacy groups, as required.
  • Communication and Liaison: Clear communication to the manager, team, and clients regarding patient case management, and liaison with healthcare providers, pharmacies, and infusion clinics, and other agencies as required.
  • Training and Development: Participate in the training process of new nurse case managers, including shadowing and call monitoring.
  • Program Development: Support and collaborate with other program staff in the development and implementation of disease management programs, including clinical requirements, writing texts, manuals, or patient pamphlets, developing compliance program parameters, needs assessments, and protocols.
  • Reporting and Compliance: Report all adverse events to the assigned units/departments, and demonstrate compliance with company policies and procedures.
Requirements
  • Education and Experience: Bachelor's degree or registered nurse (RN) diploma, advanced degree, or specific practice management experience preferred, with five (5) years of nursing experience.
  • Relevant Experience: Minimum of two (2) years in public or private third-party reimbursement arena or pharmaceutical industry in sales (not required, but an asset), managed care, or clinical support, or related experience.
  • Language Skills: Bilingualism (French and English).
  • Skills and Abilities: Strong patient focus, ability to multi-task, work under pressure, superior customer service skills, internet research and computer skills, strong verbal and written communication skills, problem-solving skills, and teamwork abilities.
