Job Summary
The Renal Care Specialist will provide direct clinical care, education, research, consultation, and treatment to support the care needs of designated renal patients. This role will function as an independent practitioner and as a collaborative partner within the inter-professional team.
Key Responsibilities
- Performing and documenting medical history and physical exam findings in the Electronic Medical Record (EMR)
- Diagnosing medical conditions
- Ordering appropriate tests/procedures in consultation with physician partners
- Interpreting laboratory/diagnostic imaging and other test results
- Prescribing pharmacologic and non-pharmacologic therapies independently or in consultation with physician partners
- Consulting as needed with the Hospitalists, Specialists, Community Primary Care Practitioners, and Allied Health team members
Qualifications
- Current registration with the College of Nurses of Ontario is required
- BScN required
- Nurse Practitioner Certification required
- Masters degree in Nursing/Nurse Practitioner Stream is preferred
- Acute Care Nurse Practitioner Certification preferred
- Gerontology Nursing Certification is preferred
- BCLS is required
