Job Summary

Northern Health Authority is seeking a highly skilled and compassionate Registered Nurse to join our Primary Care team. As a Primary Care Nurse, you will play a vital role in providing high-quality patient care and promoting health and well-being in our communities.

Key Responsibilities:

• Provide comprehensive nursing care to patients in primary care settings
• Develop and implement individualized care plans to meet the unique needs of each patient
• Collaborate with interdisciplinary teams to ensure seamless care coordination
• Educate patients and families on health promotion, disease prevention, and management
• Participate in quality improvement initiatives to enhance patient outcomes and experiences

Requirements:

• Current registration with the BC College of Nurses and Midwives as a practicing RN or RPN registrant
• Three years recent, related clinical nursing experience in primary care nursing and/or chronic disease management settings
• Current valid driver's license

Skills and Abilities:

• Assessment and Intervention: Demonstrated ability to complete initial and ongoing client assessments and provide nursing care through appropriate interventions
• Communication: Demonstrated ability to communicate effectively with clients, families, and healthcare teams
• Critical Thinking: Demonstrated ability to integrate and evaluate pertinent data to problem-solve effectively
• Person and Family Centered Practice: Ability to promote person-focused care that demonstrates care for and with people and their families
• Teaching: Knowledge of adult learning principles and facilitation skills to effectively educate patients and families
