About the Opportunity

The Nova Scotia Health Authority's Hants Health and Wellness Team is seeking a highly skilled Wellness Facilitator-Physiotherapist to join our integrated chronic disease prevention and management service.

This is an excellent opportunity for a physiotherapist with experience in working with people with chronic disease, community groups, and education facilitation. As a member of our interdisciplinary team, you will be responsible for supporting patients with the management of their chronic disease and wellness through critical thinking and clinical judgment.

Key Responsibilities:
  • Use critical thinking and clinical judgment to support patients with chronic disease and wellness in individual and group settings;
  • Collaborate with team members, primary care providers, and community groups to maintain awareness of existing supports within the community;
  • Develop and implement self-management programs and services that are responsive to community needs;
  • Maintain up-to-date knowledge of local health and community-based services.

We value candidates with strong interpersonal, teamwork, oral, and written communication skills. If you have a Bachelor's degree in physiotherapy from an accredited university, membership with the Nova Scotia College of Physiotherapists, and eligibility for membership with the Canadian Physiotherapy Association, we encourage you to apply.
