Job Title: Physiotherapist - eRehab Program

We are seeking a highly skilled Physiotherapist to join our team as a Physiotherapist - eRehab Program. As a Physiotherapist - eRehab Program, you will play a key role in providing high-quality patient care and services to our clients.

Key Responsibilities:
  • Assess and determine patient care needs and eligibility for eRehab services
  • Provide access and referrals to community services and resources
  • Engage with patients, caregivers, and other healthcare practitioners to deliver patient-centered care
  • Develop and implement treatment plans to increase occupational performance, independence, and safety
  • Utilize electronic databases and documentation platforms to manage patient records and communicate with healthcare teams
  • Establish and maintain effective relationships with multidisciplinary teams and external service providers
  • Represent Ontario Health atHome in case reviews and committees, and participate in rehabilitation meetings and workshops
Requirements:
  • Graduate of an accredited degree program in Physiotherapy (PT)
  • At least two years of recent clinical work experience as a directing physiotherapist required and current registration in good standing with the College of Physiotherapists of Ontario
  • In-depth knowledge of therapy services and other community resources in the ESC Region
  • Good understanding of the roles of healthcare professionals for eRehab services
  • Relevant physical therapy experience in a community/healthcare environment
  • Comprehensive and up-to-date knowledge of physiotherapy tools, processes, equipment, and assistive technology
  • Knowledge of funding agencies related to physiotherapy recommendations
  • Knowledge of Quality Based Procedures/Best Practices as per RCA guidelines for Stoke
  • In-depth understanding of Ontario Health atHome business priorities, objectives, and requirements for patient services
  • Practical knowledge and understanding of relevant legislation regarding the provision of healthcare services, privacy, health and safety, etc.
  • Comprehensive knowledge of the standards of practice and professional guidelines set forth by CPO
  • Shares knowledge, requests information, and collaborates with other healthcare professionals to deliver effective and efficient patient services
  • Working knowledge of computer software (email, internet) and Microsoft Office applications (Word, Excel)
