About the Role

We are seeking a skilled Physiotherapist to join our team in providing high-quality eRehab services to patients in the community.

Key Responsibilities
  • Assess and determine patient care needs and eligibility for eRehab services
  • Provide access and referrals to community services and resources
  • Engage with patients, caregivers, and other healthcare practitioners to deliver patient-centered care
  • Develop and implement treatment plans to increase occupational performance, independence, and safety
  • Utilize electronic databases and documentation platforms to manage patient records and communicate with multidisciplinary teams
  • Establish and maintain effective relationships with patients, caregivers, and healthcare professionals to ensure seamless care delivery
Requirements
  • Graduate of an accredited degree program in Physiotherapy (PT)
  • At least two years of recent clinical work experience as a directing physiotherapist, with current registration in good standing with the College of Physiotherapists of Ontario
  • In-depth knowledge of therapy services and community resources in the ESC Region
  • Good understanding of the roles of healthcare professionals in eRehab services
  • Relevant physical therapy experience in a community/healthcare environment
  • Comprehensive knowledge of physiotherapy tools, processes, equipment, and assistive technology
  • Knowledge of funding agencies related to physiotherapy recommendations
  • Knowledge of Quality-Based Procedures/Best Practices as per RCA guidelines for Stoke
  • In-depth understanding of Ontario Health atHome business priorities, objectives, and requirements for patient services
  • Practical knowledge and understanding of relevant legislation regarding the provision of healthcare services, privacy, health, and safety
  • Comprehensive knowledge of the standards of practice and professional guidelines set forth by the College of Physiotherapists of Ontario
  • Excellent written and oral communication skills, with the ability to provide mentorship and maintain professional relationships
