Dietitian, Home Health/Primary and Community Care Specialist

We are seeking a highly skilled Dietitian to join our team at Fraser Health Authority. As a Dietitian, you will play a critical role in providing consultation and nutrition education to Home Health and/or Primary & Community Care clients and staff.
Key Responsibilities- Client Assessment and Education: Conduct thorough nutritional assessments and provide education to Home Health and/or Primary & Community Care clients and families, including home visits and in-reach and/or outreach services.
- Consultation and Collaboration: Provide consultation to staff regarding clients with high-risk nutritional needs and collaborate with the interprofessional team to develop comprehensive shared patient/client plans.
- Care Planning and Coordination: Facilitate and support the transition of client care plans to referring sources, primary/community care providers, and community agencies.
- Health Counseling and Education: Provide health counseling to clients and families, including education, self-management, self-monitoring, and wellness/health promotion/prevention through a combination of clinic, telephone, or home visits.
- Resource Development and Maintenance: Develop and maintain nutrition resources for staff, including reviewing, interpreting, and evaluating professional literature and media publications.
- Community Liaison and Referral: Liaise with nutrition staff from other agencies to ensure complex nutrition needs of clients are met, and refer clients and families to appropriate community resources.
- Professional Development and Growth: Maintain professional practice growth, knowledge, and expertise by reviewing relevant literature, attending educational workshops and in-services, consulting, and networking with other healthcare professionals.
To be successful in this role, you will require:
- A Bachelor's Degree in Dietetics, plus completion of a community nutrition or dietetics internship in an accredited program or one year graduate study in nutrition and dietetics.
- Two years of recent related experience in a community or related setting.
- Current membership with the College of Health and Care Professionals of British Columbia and eligibility for membership with the College of Dietitians of Canada.
- A valid BC Driver's Licence.
