Clinical Care Coordinator

Mackenzie Health is seeking a highly skilled Clinical Care Coordinator to join our team in Adult Mental Health Central Intake Services. This role provides the first point of contact for individuals requiring outpatient mental health services, serving as an ambassador to the community.
This position plays a crucial role in providing information, guidance, and initial therapeutic mental health assessments to determine the best course of care for each individual. The Clinical Care Coordinator will work closely with colleagues and community agencies to ensure an integrated service response is accessible to those requiring support.
The ideal candidate will have a Master's degree in social work from an accredited university and be registered with the Ontario College of Social Workers and Social Service Workers or hold a General Class certificate with the College of Nurses of Ontario. A minimum of 3 years of acute mental health experience, expertise in mental health assessment, and excellent collaboration and communication skills are essential.

About the Role:
- Provide information, guidance, and initial therapeutic mental health assessments to individuals requiring outpatient services
- Coordinate appropriate Outpatient Service or community resource provision
- Facilitate and monitor all aspects of initial operational needs of clients
- Contribute to safe, quality care through monitoring and taking action with Program Manager and Coordinator
- Screen and triage referrals to the outpatient program
- Complete therapeutic mental health intake assessments
- Provide solution-focused, brief therapy interventions and lead various modalities of group therapy
- Collaborate with colleagues and community agencies to ensure an integrated service response
- Create an environment that supports professional practice, teamwork, and communication
- Liaise with members of the interprofessional team to ensure excellence in client care
- Respond to information requests through telephone, mail, and email
Requirements:
- Master's degree in social work from an accredited university and registration with the Ontario College of Social Workers and Social Service Workers or General Class certificate with the College of Nurses of Ontario
- Minimum 3 years of acute mental health experience
- Expertise in mental health assessment
- Excellent collaboration and communication skills
- Demonstrated leadership skills, sound judgment, and diplomacy
- Possess the ability to implement strategies that effectively facilitate change
- Strong problem-solving and critical thinking skills
- Ability to manage high volume and varied work assignments

Salary Range: $85,000 - $110,000 per year, depending on qualifications and experience.
