### Job Title: Patient Care Coordinator
#### Job Summary:
We are seeking a highly skilled Patient Care Coordinator to join our team at Mackenzie Health. As a key member of our program/clinical area management team, you will be responsible for the overall day-to-day coordination of patient and family care, facilitation of patient flow in and out of the ambulatory program, and coordination of clinical staff and resources.
#### Key Responsibilities:
* Coordinate and assist physicians in primary care and in Long Term Care Homes in navigating services, providing centralized access to hospital and community services, with follow-up on status of referrals, results, and consults.
* Manage a workload effectively in a fast-paced environment, addressing and resolving issues in a productive, respectful manner.
* Exercise sound professional judgment and use best practice guidelines to contribute to safe, quality care.
* Contribute to a safe, inclusive environment for all through compliance with patient and staff safety policies and procedures.
#### Requirements:
* Registered Nurse with Bachelor of Science of Nursing (BScN) Degree or currently in the process of completing the BScN.
* Current general certificate of registration and member in good standing with the College of Nurses of Ontario.
* Current Basic Cardiac Life Support (BCLS) certificate required.
#### Preferred Qualifications:
* Minimum of two years of relevant acute inpatient experience.
* Experience with QBP pathways and management of this patient population.
* Experience with case management, and discharge planning.
* Demonstrated evidence of understanding and application of principles of quality care and outcome measurement.
