Job Summary

We are looking for a skilled Occupational Therapist to assess and provide consultation, recommendations, and interventions to a diverse group of clients in their homes using evidence-based best practices. The successful candidate will be responsible for ensuring coordinated delivery of care through ongoing communication with the client care team regarding plans and schedules for interventions.

Key Responsibilities
  • Assess and implement occupational therapy interventions in accordance with professional practice guidelines and standards established by VHA and the relevant regulatory college.
  • Implement client interventions based on assessment information, client input, and in conjunction with the client's integrated care plan.
  • Ensure that services provided are best practice, evidence-based, and evaluated through the routine use of valid and reliable outcome measures.
  • Document all assessment findings, treatment plans, progress, and discharge summaries of interventions as required by the profession, referral source, and the organization.
Requirements

To be considered for this role, you must possess:

  • A degree in Occupational Therapy from a recognized school of Occupational Therapy.
  • Membership in good standing with the College of Occupational Therapists of Ontario.
  • A valid driver's license with access to a vehicle.
  • Compassionate and empathetic nature.
  • Self-driven with ability to work independently.
  • Skilled in caseload management.
  • Skilled at electronic documentation.
  • Able to provide virtual care.
