At pt Health, we are seeking a skilled Physiotherapist to join our Community Advantage Rehabilitation team in Durham Region, ON.

We are a leading provider of community-based rehabilitative therapy services, dedicated to helping adults and children maximize their functional abilities and participate fully in life's activities.

This is a flexible role, offering a range of working hours from part-time to full-time, depending on your desired client caseload size.

As a Physiotherapist with our team, you will provide ministry-funded, community-based physiotherapy services to clients in their place of residence.

Your responsibilities will include:

  • Performing physiotherapy assessments in areas such as geriatrics, neurology, orthopedics, and palliative care
  • Developing and implementing goal-oriented treatment plans in collaboration with clients and caregivers
  • Providing education to clients and caregivers, and linking them to community agencies and resources when appropriate
  • Completing and maintaining documentation related to assessment findings, care plans, progress, and discharge summaries

To be successful in this role, you will require:

  • Registration with the College of Physiotherapists of Ontario
  • Experience working in a community setting, although new graduates will be considered
  • Ability to work independently and as part of an inter-professional team
  • Excellent oral, written, and report-writing skills
  • Proficiency with computers
  • Strong interpersonal and organizational skills
  • Reliable transportation
