About the Role

We are seeking a highly skilled Care Coordinator to join our team at Home and Community Care Support Services. As a Care Coordinator, you will play a vital role in assessing and determining patient care needs, providing access and referrals to community services, and engaging with patients, caregivers, and other healthcare practitioners.

Key Responsibilities
  • Assess patient care needs and develop individual care and service plans in collaboration with patients and their families.
  • Link patients with service providers and coordinate care plan delivery.
  • Establish a helping relationship with patients and their families, balancing their needs and choices with available resources.
  • Monitor and evaluate the effectiveness of care plans and make adjustments as needed.
Requirements
  • Membership in good standing with the applicable regulatory body.
  • University degree in Nursing, Physiotherapy, Occupational Therapy, Speech Language Pathology, Social Work, or a related field.
  • 2+ years of recent experience in community health or a related field.
  • Knowledge of the healthcare delivery system and community resources.
  • Excellent interpersonal, communication, assessment, problem-solving, and decision-making skills.
  • Effective time management, prioritization, and organizational skills, with the ability to work independently and collaboratively in a busy multidisciplinary environment.
  • Established ability to accurately complete required documentation, reports, and forms.
  • A valid driver's license and access to a reliable vehicle.
  • Proficient in a Windows environment.
Preferred Qualifications
  • Experience working with diverse patient groups, including multicultural, homeless, palliative, acquired brain injury (ABI), mental health, geriatrics, and pediatrics.
  • Case management experience or recent related community experience.
  • Ability to speak French or another second language.
