About the Role:

We are seeking a compassionate and skilled Registered Practical Nurse (RPN) to join our team at Bayshore HealthCare. As an RPN, you will play a vital role in delivering high-quality home health care to our clients, ensuring they receive the care and support they need to live independently.

Key Responsibilities:

• Provide clinical care and support to clients in their homes, including wound care, medication management, and vital sign monitoring.
• Collaborate with our interdisciplinary team to develop and implement personalized care plans that meet the unique needs of each client.
• Conduct thorough assessments of clients' physical and emotional needs, identifying areas for improvement and developing strategies to address them.
• Communicate effectively with clients, families, and healthcare professionals to ensure seamless care coordination.
• Participate in ongoing education and training to stay up-to-date on best practices and industry developments.

Requirements:

• Current registration with the provincial licensing body.
• Valid driver's license and reliable vehicle.
• Graduation from an accredited School of Nursing.
