Clinical Pharmacist

We are seeking a highly skilled Clinical Pharmacist to join our team in Primary Care. As a Clinical Pharmacist, you will provide clinical pharmacy services to patients in our primary care network, working closely with an interprofessional team to identify and resolve actual and potential drug therapy problems.
Key Responsibilities
• Provide comprehensive medication management services to patients in our primary care network
• Work collaboratively with an interprofessional team to identify and resolve actual and potential drug therapy problems
• Conduct initial and ongoing patient assessments, using clinical and diagnostic reasoning to inform practice decisions
• Teach patients and others, both one-on-one and in groups, to promote patient-centered care
• Communicate effectively with patients, families, and healthcare teams, using verbal, written, and electronic communication means

Requirements
• Current registration with the College of Pharmacists of British Columbia
• A minimum of 2 years of recent, related patient care experience, ideally in a team-based primary care setting
• A valid Class 5 driver's license and access to a personal vehicle for business purposes
